In classical Urdu poetry, it describes the desolate state of the crazed lover whose garments have already been shredded to nothingness. In the context of the couplet, it implies that madness, having already stripped the lover bare, has no clothes left to rip, forcing the heart's wound itself to act as a tearable collar.
